What should a budget request include?

At minimum: who is requesting, the department and cost centre, each line item with a justification and estimated cost, the timing, the funding source, and space for approvals. The template includes all of these so requests are complete and comparable when finance reviews them.

How is this different from a purchase requisition?

A budget request seeks approval to spend against a budget, usually ahead of time and often covering several items. A purchase requisition is the request to actually buy a specific item once budget exists. Use the budget request to secure funding, then raise requisitions to spend it.
